RE: [Wellness] Becoming a Proactive Patient
I can whole heartily recommend this process. I have followed Dr. Savard's
Health record keeping system for over 5 years when Eileen was kind enough to
send me a packet then. I now go to every doctor visit: Keep in mind I have
a whole team; oncologist, neurologist, og/gyn, regular doctor and dentist I
bring a complete health history from my computer (I have it on word) Each
time I return from a doctors visit I update the info for the year including
test results, blood work, Blood pressure and pulse, medications etc. Then
each following year I just bring the updated page. I have had 4 different
Neurologists in 3 years and found re-explaining my history and making sure
they were absolutely current imperative--This works well for me (and my poor
brain) and Dr. Savards method can get you started also. Thanks Eileen and
please pass on to your sister what a wonderful thing she has done for
patients.
My best,

In response to "Becoming a Proactive Patient"..
By visiting www.mercksource.com and clicking on "You and Your Doctor" you'll
see a link for Health Form and Records. You can download these forms for
free and listen to Dr. Marie Savard's tutorial explaining how to be
proactive and get the best health care possible. Dr. Savard's forms and
tutorial were featured in the Wall Street Journal last week. There are 26
different free forms including test-results at-a-glance for women, childhood
and adult vaccinations forms and a sample letter to request your records.
